**To qualify for a $25 gift card and discount, any referral you make must be booked for an appointment and serviced by our hospital with the exception of the no charge free wellness exam. $25 Gift Card will be mailed to the referring party and a $25 discount will be given for any services billed by the hospital to the referred party.
